I should provide the contact details used in my enquiry so the operator can locate the record and verify that the request concerns my data. I should not send a passport or unnecessary information.
Consent is given through a separate checkbox that is unchecked by default and submission of the form. An enquiry cannot be accepted through this form without consent. I should not provide sensitive data, health information, payment credentials or third-party data without a lawful basis.
